At time of death

To ensure a smooth process, we kindly ask you to prepare the following information and documents:

Photos of the deceased
Full name, date, and place of birth of the deceased
Date and place of death
Parent's names, including the mother’s maiden name
Social Insurance Number (SIN) and Health Insurance Number (HIN)
Marital status (e.g., single, married, widowed, legally separated, divorced)

Date of marriage, separation, or divorce, if applicable
Names of family members to be included in the obituary
A complete set of clothing for the deceased, including undergarments, shoes, jewelry, and any personal items like perfume or makeup, if applicable
Cemetery information, such as the lot number or concession title, if pre-arranged.

Additionally, if you wish to honor your loved one with a multimedia presentation, please provide any written or audiovisual material, including photos or videos, on USB drives, memory cards, or slides.
